Health Form - REQUIRED for summer camp attendance!

     This form is required to attend summer camp.  It is a new form, different than previous years.  This form needs to be filled out and signed by a doctor and turned into camp two weeks prior to when the camper will attend camp.  Please allow time to get into the doctor.  You may fax or mail the completed form to camp. 

Summer Camp Registration Form

   There is a $10 internal registration fee if you do not register online.  Please print this application, fill it out and send it into the camp you'll be attending with your $75 non-refundable deposit.  Registrations are not complete without the deposit.

Campership/Scholarship Forms

   These forms need to be signed by a pastor and returned to camp at least two weeks prior to when camp begins.  Based on availability.  There are no camperships for off site programs (beach camp, etc...)

Retreat Reservation Request Form

   This form needs to be filled out by every adult coming to camp.  It needs to be returned to camp before you attend.
