Health Form - REQUIRED for summer camp attendance!
This form is required to attend summer camp. A new form is needed each year. This form needs to be filled out and signed by a doctor and turned into camp two weeks prior to when the camper will attend camp. Please allow time to get into the doctor. You may fax or mail the completed form to our Administration office in Frazier Park.
