Health Form

Winter Camp: There is no health form required for Winter Camp.  Each congregation should have a permission slip/health form for each camper. 

Summer Camp: We are required to have a current health form on file every year that your child attends camp.  This is so we can best care for your child while they are under our care.  This is a form that needs to be filled out and signed by a doctor.  The camper needs to have had a physical within the past two years.  Please allow time to get your child an appt. with their doctor for a physical if this is not current. 

     During the online registration process for summer camp, you will answer some basic health questions on your camper.  This is NOT the complete health form that we need to have on file.  The completed health form should be sent or faxed at least two weeks prior to when your child will be attending camp to the camp your child will be attending, NOT the administrative office. 

If your child is attending Yolijwa, Beach Camp or the Middle School Mystery Trip, send or fax the forms to Yolijwa.
